You Must Be A Member of Niche Prescriptions To Use The Mail Order Program

We Will Guarantee your Price Through Mail Order!!!!

Niche Prescriptions is delighted to make available to you the OPPORTUNITY to fill your prescription (s) by mail order in addition to filling your prescription (s) at the pharmacy.   Please check the price of your prescription at Medication Pricing – then go to your participating pharmacy with your prescription – they should honor the price.  If they do not honor the price, you can place an order for your prescription (s) through the Niche Prescriptions Mail Order Program.

 

If your pharmacy will not honor the published price for your prescription (or if you want to use Mail Order), Niche Prescriptions will honor that price through Mail Order.  There is an exclusion and two concerns identified below:

 

A.    No Prescriptions Requiring Compounding.

 

B.   Control Substance prescriptions are allowed – HOWEVER –

       they will be monitored for utilization (State restrictions will

       apply)   

                  

C.   Prescriptions requiring refrigeration – SHIPPING COST is

        significantly more than identified below – please call 1-877-

        823-1273 to find out your shipping cost

A.   You will be provided with two forms that need to be

        completed -- mailed or faxed:

        1.   Niche Prescription Registration Form (New

              Prescription) - please note that the following is

               accepted for payment – Visa, Mastercard, American

               Express, Discover, FSA/HSA Debit.

      2.Niche Prescription - Prescription (s) Refill Form

 

B.   To fill your prescription you can do one of the following:

     1.Mail the ORIGINAL prescription to:

           a.The address will be provided once you join Niche

                   Prescriptions.

      2.   A telephone number for your physician to fax will be

                provided after you have joined Niche Prescriptions.

     3.EPRESCRIBE instructions for your physician will be

             provided after you join Niche Prescriptions.

                                    

C.   Payment:

  1.You will be paying the mail order company directly for

              your prescription (s) therefore you will have to have a

              credit card/debit card on file with them after joining

              Niche Prescriptions you will be given instructions

                  to how to do this

       2.    If your prescription includes refills – your refills will

                 be included as part of your prescriptions.

 

D.   Members will be the charged the published prescription

        price plus usual first class 2 -5 day shipping charge of $4.99

        for up to 2 prescriptions. Members with 3 or more

        prescriptions that fit in a United States Postal Service Flat

        (medium flat box- 11" X 8.5" X 5.5) box the charge is $8.99.
